Purpose:

These documents enables a California resident who is the parent of any minor child(ren) (i.e., anyone under 18 years of age) to nominate guardians to take the legal custody of such minor(s) if the parent is incapacitated and also after death of the parent.

Document Only Fee: $200

Add-ons:

• Witness Services ($25). We will print your Guardianship Declaration on premium paper, and witness the signing at our Fremont office during normal business hours by prior appointment.
